I'm building a "standard" PC with an AMD 64 CPU and need a reliable
power supply, preferably under $70 and 400-600 watts. I researched
Antec SmartPower and read a few reviews about DOAs and the PS only
lasting a few months. What about Antec EarthWatts? I am trying to
limit the cost of the case and P/S to $100 max and don't care too much
about the case. If the case can accommodate 120mm fans that is a big
plus.

Apparently the reviewers think this a pretty good PS as well.
$60 including shipping, after the rebate.

Cases are a matter of personal taste and what you want to settle. I've used
Enlight cases as well as Cooler Master cases.
One Cooler Master case is this

though it's $65 with shipping.

Unless you'll have a higher-end gaming video card installed,
it's unlikely you'd need the upper end 600W, 400W-500W
should be enough and are more common at good prices.

Unless a PSU looks like it was crudely assembled (and that
contributing to the fault), any PSU will have a small # that
are DOA. Since Antec is such a popular brand, for a time
even the most popular in the retail market, it is difficult
to know if the Antec failure rate is higher or just
represents more units sold.

EarthWatts is new enough that nobody has a good idea of
long-term viability, but it's made by Seasonic and seems a
reasonable choice.

Have a look at the Antec Sonata II then. I think you get the Earthwatts PSU
with it and a 120mm fan in the back.
